LILRR ADOPTION AGREEMENT
Terms Of Adoption
1. Transfer of Ownership: THIS RESCUED LABRADOR RETRIEVER MUST
NOT BE SOLD
OR GIVEN AWAY. If for any reason this Adopter is unable to keep this
Labrador Retriever, LILRR requires the dog be returned to a prearranged
LILRR Rescue Representative by Adopter or at Adopter's expense. This dog's
rabies tag, state dog license & tag, AKC registration and microchip registry
certificates, all vet records and supplies loaned by LILRR for Adopter's
temporary use must be returned with the dog.
1.A Adopter agrees to timely provide LILRR change of
residence and phone
during dog's lifetime.
2. Non-breeding: Rescued dogs are not to be bred. This
Lab has been
neutered/spayed.
3. Identification: This dog is microchipped for identification prior
to
adoption and LILRR will remain alternate contact with registry. Adopter
agrees to license this Labrador in accordance with existing laws in the
Adopter's town/state, and that such license tag(s), microchip tag, rabies
tag, LILRR tag and home/owner ID tag be secured on collar this dog will wear
at all times outside Adopter's home.
4. Health: This dog's health and medical history, as known by
LILRR has
been fully disclosed to Adopter. All warranties, including implied
warranties of merchantability and fitness for a particular purpose, are
hereby specifically disclaimed.
4.A Adopter agrees to take this dog to a licensed
veterinarian within
ten (10) days from this date and dog continue full inoculation and health
maintenance program as prescribed. Adopter agrees to seek immediate
veterinary aid should this dog become ill or injured and follow veterinary
recommendation for effective recovery. Should this dog become suspect of
serious health problems Adopter agrees to obtain diagnosis by board certified
veterinary specialist and follow recommendation to restore and maintain dog's
health and quality of life.
5. Care: Adopter accepts full responsibility for continuing
care of this
dog to include safe confinement, nutritional diet, continuous clean water
supply, health maintenance, shelter, sanitation, exercise, socialization, and
training. An obedience course is recommended to establish respect and
close
rapport more rapidly between the Lab and Adopter. If the dog has already
been trained, a "refresher" course in obedience is still recommended.
Adopter agrees to obtain professional evaluation and corrective training
should dog indicate problem behavior.
5.A Adopter agrees to provide this Lab a loving home
where the human
hand is always the deliverer of pleasant experiences.
5.B Adopter agrees to give this Lab first consideration
when other
animals are brought into the home.
5.C Adopter agrees to ascertain no one abuse, harm or
endanger this Lab
and insist that all children, household members, guests and strangers treat
this Lab with kindness and respect.
5.D Adopter agrees this Lab will not be transported
uncrated or
unharnessed in an open vehicle, and never in an open truck bed.
6. Control: This dog will not be allowed to run loose or stray
off
Adopter's property. Adopter agrees: to abide State and local ordinances
and
leash laws; this Lab will be on leash when not on Adopter's property and/or
other unfenced private or public property; this Lab will never be kept
confined by dog pen/run, chain, rope, "trolley" or "runner" method; this
Lab will never be
left outside unsupervised by a responsible adult; this Lab will be kept
inside the home when no adults are home and shall sleep inside the home
nights.
6.A If this Lab becomes lost or stolen Adopter agrees
to immediately
notify LILRR and make every effort to find this dog including placing ads in
newspapers; notify animal control, shelters, police, pet-finding services and
rescue organizations; post and distribute bulletins and posters and
physically check county and town animal shelters at least twice weekly.
7. Communication: LILRR welcomes communication at anytime for
reasons
regarding this dog and adoption. Adopter agrees to update LILRR on this
dog
weekly for the first month; monthly for the first six (6) months and annually
thereafter.
8. Death: Adopter agrees to inform LILRR when this dog passes
away and to
provide for a respectful and humane interment of the body remains.
9. Adoption Fee: The adoption fee is understood and agreed to
imburse
basic veterinary expenses, ID & registry fee, animal shelter and state
license fees incurred by LILRR for this dog (receipts provided Adopter at
time of adoption).
10. Refund Policy: If this dog is returned safely and alive to LILRR
within
fourteen (14) days of the agreement a full refund of adoption fee will be
provided to the Adopter less any portion required for medical services to
treat illnesses or injuries that occurred while this Lab was in the Adopter's
care. No refunds are provided after the fourteen day period has lapsed.
11. Donation: Donations are tax deductible to the extent of the law,
directly applied to ongoing rescue efforts and not refundable.
CONDITIONS: By application and signature, Adopter attest no member of
household is known to have canine related asthma or allergies. Adopter/s
acknowledge receipt and understanding and covenant to abide the terms and
conditions of this binding contract.
RESERVATION OF RIGHTS: LILRR reserves all rights to an ongoing review of
this adoption and to follow up reports or complaints to protect this Labrador
Retriever's welfare. If these terms are not upheld and/or if Adopter made
misrepresentation in application or verbally to obtain this dog, LILRR
reserves the right to void this contract and demand immediate safe return of
this dog without resort of court action. If a violation of this contract
results in legal suit, Adopter/s agree to indemnify all legal and court fees
incurred.
COMPLETE RELEASE OF LIABILITY AND INDEMNIFICATION: The undersigned
Adopter/s
acknowledge full disclosure of this dog's health, temperament and history as
known by LILRR has been made and agrees to accept full responsibility for the
behavior of this Labrador Retriever chosen for adoption. Adoper/s agree to
release and indemnify and covenants to hold harmless all Long Island
Labrador Retriever Rescue, Inc. representatives and volunteers from all
claims of liability or damages for conduct of this Labrador Retriever on and
after the date of this adoption. The undersigned Adopter/s further agree
and
intends this release of liability and indemnification shall apply to all
known, unknown, and unanticipated damages resulting from my/our adoption,
ownership, and control of such said animal.
